Septic Authorization 555-20-008854-AUTH Page 2 of 2 <br /> Date Issued:5/10/21 Date Expiring:5/10/22 <br /> Work Description: 'FIRE:AUTH 2 BDRM TO REPL 2 BDRM SAME'LOC <br /> Conditions of Approval: <br /> THIS SYSTEM IS AUTHORIZED FOR RECONNECTION UNDER OAR 340-071-0205(4)NO INCREASE IN FLOW. <br /> Marion County is simultaneously issuing the Certificate of Satisfactory Completion for a Minor Repair at this property dated <br /> 5/10/2021.The repairs that were needed to the existing system have been finished and the septic system is now <br /> approved for reconnection.The system consists of a 1000 gallon concrete septic tank and 40 lineal feet of drainfield as <br /> detailed on the as-built form for this property dated 4/16/2021.Repairs were performed by Ken Tooze Excavation. <br /> FLAG OR CORDON OFF THE DRAINFIELD AREA,IT IS IMPORTANT FOR THE INTEGRITY OF THE SYSTEM THAT IT <br /> NOT BE DRIVEN ON BY HEAVY EQUIPMENT OR VEHICLES DURING CLEANUP. <br /> This parcel was involved in the 2020 Beachie Creek Fire and the house was destroyed. <br /> There will be no increase in flow as the 2 bedroom dwelling will be replaced with a new 2 bedroom dwelling.Under OAR <br /> 340-71-0205(4),this system is not currently failing and meets current setbacks. It is AUTHORIZED for reconnection. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic including construction vehicles, <br /> livestock activities.Other activities such as logging,excavating or filling,driveway placement or any similar activities may also <br /> damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future <br /> repair system must both be protected. <br /> The septic tank should be pumped and inspectedby a licensed septic tank pumping service approximately every five(5)years. <br /> The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid <br /> materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible <br /> damage to the drainfield and will eventually lead to a system failure. <br /> Note:This Notice does not guarantee satisfactory or continuous operation of the sewage system.Should the system fail,a repair <br /> permit from DEQ is required. <br /> If you disagree with this report,you have the right to apply for an authorization notice denial review.The application for'review <br /> must be submitted in writing within 45 days of the report issuance and be accompanied by the review fee in OAR <br /> 340-071-0140(3),Table 9C and any additional information DEQ needs to complete the review. <br /> You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of <br /> the site evaluation report, plans and specifications for the proposed system,specify the rule(s)to which a variance is being <br /> requested,demonstrate the variance is warranted,and include the variance fee in OAR 340-071-140 Table 9C. A variance may <br /> only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions <br /> render strict compliance unreasonable,burdensome or impractical.
