y Building Inspection Division amort County Onsite Wastewater Review OREGON PUBLIC WORKS Activity Number: 555-22-000778-AUTH Date: March 11,2022 Project Address: 5462 Ahrens Rd SE, Salem, OR 97317 082W2000402 Completed By: Aidan Bammes,EHST ONSITE WASTEWATER TREATMENT SYSTEM AUTHORIZATION NOTICE Temporary Personal Hardship Existing system information: Marion County records indicate a minor alteration and an authorization for an increase in flow to the septic system serving this property filed in 2020.The system was installed in 1989 with 375 lineal feet of drainfield and a 1000-gallon combo tank that was installed in 2020(555-20-001095).The onsite septic system services a 3-bedroom primary residence and will serve an additional 1-bedroom hardship residence. Reference the attached stamped site plan. The tank is new(installed 2020)and has not yet required pumping.It will require pumping before February of 2030. THE AUTHORIZATION NOTICE FOR CONNECTION TO A TEMPORARY HARDSHIP DWELLING TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site plan dated March 11, 2022. PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS, A SITE VISIT IS REQUIRED EVERY 10 YEARS MINIMUM. THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service approximately EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic,livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The well placement is especially critical and must be located at least one-hundred(100)feet from the approved initial and replacement DRAINFIELD areas.
