Reference the As Built dated October 5, 2011. The hardship dwelling designation was established in
2011 during which time a pump tank was placed to reach the existing drainfield.
The current system consists of a 1000-gallon tank for the primary home,a 1500-gallon combination dosing septic
tank for the hardship dwelling, and 225 linear feet of drainfield. The tank was pumped by Ace Septic on July 18,
2016;it will need to be pumped again before July of 2026.Photos of the drainfield area and tanks were supplied by
the applicant,no signs of failure were visible. The site was visited by Marion County Onsite Septic in 2016 for the
previous renewal. The next renewal will require a site visit.
THE AUTHORIZATION NOTICE TO CONTINUE CONNECTION TO A TEMPORARY HARDSHIP
DWELLING TO THE EXISTING ONSITE SYSTEM IS APPROVED. Please refer to the signed approved site
plan dated March 11, 2022.
PERSONAL HARDSHIP RENEWAL IS REQUIRED EVERY 5 YEARS,A STl'h VISIT IS REQUIRED EVERY
10 YEARS MINIMUM.
THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping service
approximately EVERY FIVE(5)YEARS. The septic tank is a reservoir for the solid materials in the sewage waste
stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the
tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a
system failure.
THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and
farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar
activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the
initial septic system and the future repair system must both be protected.
OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. The well
placement is especially critical and must be located at least one-hundred(100)feet from the approved initial and
replacement DRAINFIELD areas. A copy of the DEQ setback requirements is available in our office.
555 Court St.NE — P.O.Box 14500—Salem,Oregon 97309-5036—Phone(503)588-5147—Fax(503)588-7948
Page 1 of 1
