Septic Authorization 555-17-004859-AUTH Page 2 of 2 <br /> !T� �- <br /> Conditions of Approval: • <br /> Existing system information: <br /> Marion County records show the existing septic system serving this property was installed In 2002 under permit <br /> number 02-04555. The pressurized distribution system consists of a 1500 gallon concrete combo tank and 225 <br /> linear feet of drainline. A Certificate of Satisfactory Completion is on file. The system has reportedly never <br /> been used. <br /> A site visit was made to the property on July 10,2017. The area of the drainfield is relatively undisturbed. The <br /> repair area is intact and free of disturbance. Both areas lie in partially wooded areas that are free of roadways. <br /> Flow Increase information: <br /> The proposal is to connect a 3-Bedroom dwelling to this system that was designed for a flow of 450 gallons per <br /> day. This AUTHORIZATION IS APPROVED. Please refer to the signed approved site plan dated August 24, <br /> 2017. <br /> • THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic including construction <br /> vehicles, livestock and farming activities. Other activities such as logging, excavating or filling,driveway <br /> placement or any similar activities may also damage the soil in the area and adversely affect the septic system <br /> approval. The areas of the initial septic system and the future repair system must both be protected. <br /> The septic tank should be pumped and inspected by a licensed septic tank pumping service approximately <br /> every five(5)years. The septic tank is a reservoir for the solid materials in the sewage waste stream. <br /> Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank <br /> and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a <br /> system failure. <br /> Note: This Notice does not guarantee satisfactory or continuous operation of the sewage system. Should the <br /> system fail, a repair permit from DEQ is required. <br /> If you do not agree with this report, you have the right to apply for an authorization notice denial review.
