Conditions observed around the test pits are approved for a STANDARD <br /> system for the alteration and for the REPAIR. Plans are approved as revised. Please <br /> refer to the signed approved site plan dated September 10, 2019. Any deviation from <br /> the approved plans must be authorized by this office prior to installation or the permit <br /> may be voided. It is the responsibility of applicant/installer to coordinate these <br /> requirements with all others in OAR 340-071-0220. <br /> This approval is for a system designed to serve a single family dwelling with a <br /> maximum of 4 bedrooms. Peak sewage flow into the system is limited to a maximum <br /> of 450 gallons per day, with an average sewage flow of not more than 225 gallons per <br /> day. Premature failure of the treatment system may occur if either of these flow <br /> quantities is exceeded. If, for some reason, you expect your domestic household <br /> water use may exceed these flows, it is advisable to increase the size of the treatment <br /> system. <br /> Specific Requirements <br /> EXISTING TANK IF APPLICABLE + 500 GALLON CONCRETE DOSING TANK <br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal <br /> field in the approved area and at the required depth. Detailed instructions for dosing <br /> tank/effluent pump installations are available in our office. The electrical wiring to the <br /> pump and alarm must be installed in accordance with the appropriate Electrical Code. <br /> The electrical work must be done under a Marion County Electrical Permit and must be <br /> inspected and approved by a Marion County Electrical Inspector. The work must be <br /> left uncovered and accessible for inspection. The electrical inspection includes the <br /> connections to the pump and alarm in the dosing tank, the wiring in the trench and the <br /> connections in the structure. The effluent pipe must have 18-gauge minimum green <br /> tracer wire or green metallic tape placed along the line. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a <br /> minimum of 4 feet, must be installed level, and bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the <br /> Department of Environmental Quality rules.
