2-YEAR AX20N SERVICE CONTRACT PRESSURE DISTRIBUTION DRAIN FIELD UV SCHEDULE A ❖ Total Fee to be paid to A&B Septic Service by the installer,or directly by the customer at time of inspection and initial Start-up by 0& M Provider. •:• Contract is void, if not signed within 30 days from October 26, 2021 ❖ Contract is void, if County does not issue installation permit. •: Emergency call outs; unscheduled site visits because of alarm; Lab Testing; Cleaning of Filter Media; Parts not covered by warranty;or Pumping of Tanks will be billed at Time and Materials. •: Telephone line should be connected to VeriComm Telemetry Panel by start up to properly monitor the system. When the panel is connected to the computer by a phone line and has an alarm we are notified at our office and are able to look at your system and make minor adjustments at no charge. If the system is not hooked up to a telephone we may need to make a site visit to determine the reason for the alarm. ❖ GENERAL TERMS AND CONDITIONS FOR A&B SEPTIC SERVICE. Customer agrees to pay all sums due and owing to A&B Septic Service in a timely manner. Interest shall accrue on all past due sums at the rate of 18% per annum. Customer also agrees to pay A&B Septic Service for all costs relating to actions to collect past due sums, including reasonable attorney fees in any forum regardless of whether a lawsuit is filed. Customer also agrees to release, hold harmless, and indemnity A&B Septic Service from any and all damages to persons and real and personal property arising out of or resulting from services rendered by A&B Septic Service ❖ If a phone line is not going to be installed then we will download the last 90 days information when we do our scheduled maintenance to update the history in the main VeriComm computer. • When filing the 2nd Year Annual report to Regulatory Authority we must notify them if a new extended service contract is not in place. ❖ Thank you for choosing A&B Septic Service we appreciate your business. *Payment for the 2 year service contract is due and payable with the return of the signed contract. The fee covers the following: • Creating documentation, records preparation, files and computer entry • Site visit to do initial start-up of system & reporting to the Regulatory Agency •:• Service visits at approximate 6 month intervals (total of 4) • Field analysis&testing supplies •: Annual Operation & Maintenance Report sent to the customer&Regulatory Agency • Telemetry monitoring(if connected to a phone line or Ethernet connection) :• The fee is non-refundable, however if you sell the property the contract and any fees paid can be transferred to the new owner.
