PERFORMANCE OF SERVICES <br /> Bennett Septic Service LLC shall perform system inspection/service visits during the two-year period after installation,as shown <br /> below. <br /> INSPECTION/SERVICE VISITS <br /> *3 to 6 months *6 to 12 months *12 to 18 months *18 to 24 months <br /> ALARM RESPONSE OTHER SERVICES <br /> *System Monitoring *Alarm Response *Reporting <br /> Re orti <br /> P n8 <br /> As required by NSF International,these services will be included as part of the initial purchase of the system for the first two years <br /> of system operation. On this date _/_J ;the Whitewater System 2 year Start Up contract reaches its maturity. The <br /> subsequent service contract would need to be signed between the homeowner and the service provider to continue to receive <br /> services.The subsequent service contract will follow manufactures requirements in the Whitewater Service Manual. <br /> Inspection/Service visits shall be performed during normal business hours Monday through Friday (excluding holidays) on a <br /> prescheduled basis and as the authorized Whitewater service provider deems necessary or advisable. <br /> At each service visit,the Whitewater system shall be inspected and serviced in accordance with the instructions in the Whitewater <br /> System Service Manual. Additionally, an effluent quality inspection consisting of an olfactory assessment for odor, a visual <br /> assessment for scum overflow,color and turbidity shall be performed.Other operational diagnosis or performance assessment <br /> may be deemed necessary to determine and/or ensure operation proper of the facility.This include tysampling <br /> effluent(post-disinfection when disinfectant is needed).If the facilityhas a UVmay p g of treatedp <br /> light system in place,the UV light <br /> and/or cleaned g will be replaced <br /> / 1 aned in accordance with manufacturer requirements.Any additional charges(parts and/or labor)associated with th e <br /> UV light system will be billed to the homeowner. <br /> The service provider shall provide emergency service within 48 hours of a service re <br /> quest. We may need to make <br /> n site <br /> visit to determine the reason <br /> for the alarm. (Site visits will be billed to the owner) <br /> The service provider shall also be responsible for submitting the annual report. The annual evaluation fee to the appropriate <br /> regulatory agency shall be paid by the owner of the system. <br /> REPROTING WILLBBBILLEDAT TIME OFRRPORTTIITG: <br /> PRICE OF THIS 2-YEAR CONTRACT IS$800.00. <br /> The service provider shall notify the homeowner in writing if any improper system operation cannot be remedied at the time of <br /> servicing. The written notification shall include an estimated date if correction. <br /> PERMITS <br /> If permits are required,the homeowner would be responsible for the associated costs payable to the service <br /> provider before the permit application is submitted. <br /> DEFINITIONS <br /> For purposes of this service contract, the following definitions shall apply: <br />
