Based on the results of this study, site evaluation approval is granted. The approval is site specific to the area tested, and the septic system must be located in the immediate area of the test pits, unless otherwise authorized by this office. A new site evaluation may be required if the system is proposed for a different area on the property. This approval is given on the basis that the lot or parcel will not be further partitioned or subdivided and that conditions on this property and on adjacent properties are not altered in any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be altered or modified by cutting, filling, or other developmental activities which would impede the successful operation of the initial or replacement soil absorption systems. Failure to comply with the above items may void this approval. This report is not a permit to install the sewage system. A permit must be obtained before any system construction occurs. A permit may be issued only after the property has been found to be compatible with applicable LCDC acknowledged local comprehensive land use plan. A "Statement of Land Use Compatibility" or its equivalent is required before a construction permit can be issued. A detailed site plan, drawn to scale (ex: 1" = 60"), must be submitted with the permit application before the construction-installation permit can be issued. The system must be installed by the property owner or a DEQ licensed installer. This site evaluation report is valid until an on-site sewage system is installed pursuant to the construction permit obtained from this office, or until cancelation pursuant to the Environmental Quality Commission Rules. Subject to the above, this report runs with the land and will automatically benefit subsequent owners. The trenches are perforated pipe and are installed level (+/- one inch) and contoured to follow the natural ground surface. Before any installation takes place, a permit must first be obtained from Marion County. See specific review requirements on attached.document.
