EXISTING SYSTEM APPROVAL DATE/TIME : 12/21/98 15:23 PERMIT NO : 98-10190 TYPE : SPMS STATUS : ISSUED OCCUPANCY : R-3 ISSUED : 12/21/1998 TO EXPIRE : 12/21/1999 PAGE 2 EXISTING SYSTEM APPROVAL A site inspection was made to determine the condition of the existing on-site sewage system located on the property. No evidence of failure was found and.the system appeared to be functioning in a satisfactory manner. The septic tank was pumped and inspected on 12/ 1/98 by A&B SEPTIC SERV. This notice is issued with the recommendation that the septic tank be pumped and inspected by a licensed septic tank pumper approximately every five years. This notice does not guarantee satisfactory or continuous operation of the existing on-site sewage disposal system. Planning approval may also be required for any proposed development of the property. COMMENTS AND CONDITIONS 1. CALL BEFORE YOU DIG....IT'S THE LAW. ATTENTION: Oregon law requires you to follow rules adopted by the Oregon Utility Notification Center. Those rules are set forth in OAR 952-001-0010 through OAR 952-001-0090. You may obtain copies of the rules by calling the center. (Note: The telephone number for the Oregon Utility Notifica- tion Center is (503) 232-1987). 2. THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 12/14/98. THE EXISTING SEPTIC TANK WAS PUMPED AND CERTIFIED TO BE IN GOOD CONDITION, IT WAS INDICATED BY THE TANK PUMPER THAT IT WAS NOT NECESSARY TO PUMP THE TANK AT THIS TIME. THE HOUSE AD- DITION WILL NOT CONFLICT WITH THE EXISTING SEPTIC SYSTEM. THERE IS ROOM AVAILABLE ON THE PROPERTY FOR FUTURE DRAIN- FILD REPLACEMENT. 3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling. driveway placement or any similar activities may also damage the soil • in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected.
