No evidence of failure was found and.the system appeared to be <br /> functioning in a satisfactory manner. The septic tank was pumped and inspected on 12/ 1/98 <br /> by A&B SEPTIC SERV. <br /> This notice is issued with the recommendation that the septic tank be pumped and inspected by <br /> a licensed septic tank pumper approximately every five years. This notice does not guarantee <br /> satisfactory or continuous operation of the existing on-site sewage disposal system. <br /> Planning approval may also be required for any proposed development of the property. <br /> COMMENTS AND CONDITIONS <br /> 1. CALL BEFORE YOU DIG....IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 232-1987). <br /> 2. THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 12/14/98. THE <br /> EXISTING SEPTIC TANK WAS PUMPED AND CERTIFIED TO BE IN GOOD <br /> CONDITION, IT WAS INDICATED BY THE TANK PUMPER THAT IT WAS <br /> NOT NECESSARY TO PUMP THE TANK AT THIS TIME. THE HOUSE AD- <br /> DITION WILL NOT CONFLICT WITH THE EXISTING SEPTIC SYSTEM. <br /> THERE IS ROOM AVAILABLE ON THE PROPERTY FOR FUTURE DRAIN- <br /> FILD REPLACEMENT. <br /> 3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br /> 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling. driveway <br /> placement or any similar activities may also damage the soil <br /> • in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />
