NOW,THEREFORE,in consideration of the terms,provisions,covenants and conditions contained <br /> herein,the Parties hereto agree as follows: <br /> 1.0 Performance of Basic Services <br /> The Authorized Service Provider,shall perform the System Inspection/Service Visits during the 24-month <br /> period after installation,as marked: <br /> Inspection/Service Visits 3-6 months X <br /> 6-12 months X <br /> 12-18 months X <br /> • 18-24 months X <br /> Alarm Response <br /> Other Services Reporting X <br /> These services shall be performed during normal business hours Monday through Friday(excluding <br /> national holidays)on a pre-scheduled basis and as the Authorized Service Provider deems necessary or <br /> advisable.The maintenance inspection will include the following services at each 6 month visit: <br /> a) Visual inspection of the system operation to confirm water tight tankage,verify proper <br /> pump,blower,float switch,control panel and audio visual alarm operation(as applicable) <br /> and absence of effluent on the ground surface at all component locations including the <br /> disposal field <br /> b) Measure and record residuals thickness(scum and sludge) <br /> c) Visual inspection of filter media and physical cleaning of components if required <br /> d) Sensory examination of treated effluent for clarity,odor,oily sheen,foaming or any other <br /> unusual characteristics <br /> Each inspection of the equipment shall be followed by a written report to the homeowner. The <br /> inspection report will describe the operational status of the system and any improper system function <br /> that was not remedied during the time of inspection and include an estimated date of correction. It will <br /> also include recommendations for any preventative maintenance deemed necessary by the <br /> maintenance provider as well as a list of any replacement parts needed. <br /> The owner must report evidence of any system failures to the agent and take appropriate action <br /> approved by the agent to correct the problem. <br />
