NOW,THEREFORE,in consideration of the terms,provisions,covenants and conditions contained herein,the Parties hereto agree as follows: 1.0 Performance of Basic Services The Authorized Service Provider,shall perform the System Inspection/Service Visits during the 24-month period after installation,as marked: Inspection/Service Visits 3-6 months X 6-12 months X 12-18 months X • 18-24 months X Alarm Response Other Services Reporting X These services shall be performed during normal business hours Monday through Friday(excluding national holidays)on a pre-scheduled basis and as the Authorized Service Provider deems necessary or advisable.The maintenance inspection will include the following services at each 6 month visit: a) Visual inspection of the system operation to confirm water tight tankage,verify proper pump,blower,float switch,control panel and audio visual alarm operation(as applicable) and absence of effluent on the ground surface at all component locations including the disposal field b) Measure and record residuals thickness(scum and sludge) c) Visual inspection of filter media and physical cleaning of components if required d) Sensory examination of treated effluent for clarity,odor,oily sheen,foaming or any other unusual characteristics Each inspection of the equipment shall be followed by a written report to the homeowner. The inspection report will describe the operational status of the system and any improper system function that was not remedied during the time of inspection and include an estimated date of correction. It will also include recommendations for any preventative maintenance deemed necessary by the maintenance provider as well as a list of any replacement parts needed. The owner must report evidence of any system failures to the agent and take appropriate action approved by the agent to correct the problem.
