Onsite Wastewater Review <br /> ° . 0 KON,iimi <br /> Pl1BLi .:WORKS <br /> Activity Number:555-24-006535-INQY Date: September 10th,2024 <br /> Project Address: 5133 Center ST NE, Salem <br /> 4.3-Acres of 092W120001200 from PLA 22-029 <br /> Completed By: Matt Knudsen <br /> SEPTIC SYSTEM RECORD REVIEW <br /> This record review was conducted as the property is getting smaller and to determine that there is enough <br /> room for a repair system, when needed. <br /> The Existing System Evaluation Report document created by Fuhriman Septic Design and Consulting <br /> shows that the system consists of an 1000-gallon tank and 150 linear ft of tile drainline. A site plan and <br /> existing system evaluation report was submitted on 08/16/2024 showing the system is located entirely <br /> within the proposed parcel meeting setbacks to the property lines. There appears to be room for a repair <br /> area when needed. <br /> THIS RECORD REVIEW IS BASED ON THE INFORMATION SUBMITTED BY THE APPLICANT. <br /> THE SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic,livestock and farming <br /> activities. Other activities such as logging, excavating or filling, driveway placement or any similar <br /> activities may also damage the soil in the area and adversely affect the septic system approval. The areas <br /> of the initial septic system and the future repair area must both be protected. <br /> THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank pumping <br /> service approximately EVERY FIVE (5) YEARS. The septic tank is a reservoir for the solid materials in <br /> the sewage waste stream. Excessive accumulation of these solid materials in the septic tank can cause the <br /> solids to wash out of the tank and into the drainfield.
