SE
Aumsville, Oregon 97325

An inspection was made at the above address on June 3, 1987 to
determine if the existing on-site sewage system located on the
property appeared adequate to serve the purpose described in the
attached application. This application is to place an addition
onto an existing house. No evidence of failure was found and the
system appeared to be functioning in a satisfactory manner. The
septic tank was pumped on March 19, 1987 by Stayton Septic
Service and the tank was inspected at that time.

This notice is issued with the recommendation that the septic
tank be pumped and inspected approximately every five years. This
notice does not guarantee satisfactory or continuous operation of
the existing on-site sewage disposal system.

COMMENTS:

The septic system was installed and inspected in June
of 1977. The addition cannot be placed over the
drainfield. If any portion of the drainfield is located
under the proposed addition, it will be necessary to
obtain a septic repair permit and relocate that portion
of the drainfield.

Our office
contact our office if you have any questions
repo~~

hours are Monday through Friday, 8:00 to 4:30.
