The conditions found on the property do not meet <br />the requirements of the Department of Environmental Quality On- <br />Site Sewage Disposal Rules and your site evaluation application <br />is denied. Listed below are the site conditions and the cor- <br />respon~ requirements of the D.E.Q. rules. <br /> <br />1. Permanent water table at 1. The denial review fee is <br />sixty dollars ($60) and the application must be submitted to the <br />Willamette Valley Region Office of D.E.Q. within thirty (30) days <br />of denial notification.
