This application is to replace an existing
mobile home. No evidence of failure was found and the system
appeared to be functioning in a satisfactory manner. The septic
tank was pumped on September 1, 1987 by Stayton Septic Service
and the tank was inspected at that time.

This notice is issued with the recommendation that the septic
tank be pumped and inspected approximately every five years. This
notice does not guarantee satisfactory or continhous operation of
the existing on-site sewage disposal system.

COMMENTS:

We have no record of the existing septic system in our
files. The existing home is vacant and the septic
system is not in use at this time.

Our office hours are Monday through Friday, 8:00 to 4:30. Please
contact our office if you have any questions concerning this
report.

ROBERT FOSTER, REGISTERED SANITARIAN
