This application is to replace an existing
mobile home. No evidence of failure was found and the system
appeared to be functioning in a satisfactory manner. The septic
tank was pumped on June 19, 1986 by Stayton Septic Service and
the tank was inspected at that time.

This notice is issued with the recommendation that the septic
tank be pumped and inspected by a licensed septic tank pumper
approximately every five years. This notice does not guarantee
satisfactory or continuous operation of the existing on-site
sewage disposal system.

COMMENTS: This system was inspected in 1986 and has had little
use since that time. An easement has been obtained for
the existing drainfield. This application is for a one
bedroom home.

Our office hours are Monday through Friday, 8:00 to 4:30. Please
contact our office if you have any questions concerning this
report.~. ~

ROBR~T~RT FOSTER,' REGISTERED SANITARIAN
