This application is to remodel and finish base-
ment. No evidence of failure was found and the system
appeared to be functioning in a satisfactory manner.
The septic tank was pumped on February 1, 1989 by
Mike's Septic Service. and the tank was inspected at
that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed
septic tank pumper approximately every five years. This
notice does not guarantee satisfactory or continuous
operation of the existing on-site sewage disposal
system.

COMMENTS: The septic system was installed and inspected
in August of 1970 and consists of a 900
gallon septic tank and 250 lineal feet of
drainfield. The system sizing was adequate
for a three bedroom under the rules in effect
when the permit was issued in 1969.

Our office hours are Monday through Friday, 8:00 to
4:~ Please contact our office if you have any
qu~n s~report '__
ROBERT FOSTER, REgiSTEreD SANITARIAN

Senator Building 220 High Street NE Salem, Oregon 97301-3670
