The result~ of that inspection and specifi- <br />cations for the sewage system are on the attached re- <br />ports. The approved sewage disposal area includes area <br />for the initial sewage system and full replacement area <br />to be used in the event of the failure of the initial <br />system. <br /> This approval is given on the basis that the lot or <br />parcel will not be further partitioned or subdivided and <br />that conditions on this property and on adjacent proper- <br />ties are not altered in any manner which would prohibit <br />the issuance of a permit. Soils in the approved area <br />shall not be altered or modified by cutting, filling, or <br />other developmental activities which would impede the <br />successful operation of the initial or replacement soil <br />absorption systems. Failure to comply with the above <br />items may void this approval. <br /> This report is not a permit to install the sewage <br />system. A permit must be obtained before any system <br />construction occurs. A permit may be issued only after <br />the property has been found to be compatible with appli- <br />cable LCDC acknowledged local comprehensive land use <br />plan. A Statement of Land Use Compatibility or its equiv- <br />alent is required before a construction permit can be <br />issued. <br /> A detailed plot plan of the proposed system must be <br />submitted with the permit application before the con- <br />struction-installation permit can be issued. The system <br />must be installed by the property owner or a DEQ licensed <br />septic installer. <br /> This site evaluation report is valid until an on-site <br />sewage system is installed pursuant to the construction <br />permit obtained from this office, or until earlier can- <br />cellation pursuant to the Environmental Quality Commis- <br />sion rules. Subject to the above, this report runs with <br />the land and will automatically benefit subsequent own- <br />ers. <br /> Our office hours are Monday through Friday, 8:00 to <br />4:30.
