Box 484
Detroit, Oregon

An inspection was made at the above address on October
5, 1990 to determine if the existing on-site sewage
system located on the property appeared adequate to
serve the purpose described in the attached applica-
tion. This application is to replace a mobile home on
the property. No evidence of failure was found and the
system appeared to be functioning in a satisfactory
manner. The septic tank was pumped on April 26, 1983 by
Stayton Septic Service and the tank was inspected at
that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed
septic tank pumper approximately every five years. This
notice does not guarantee satisfactory or continuous
operation of the existing on-site sewage disposal
system. Planning approval may be required for the pro-
posed development of the property.

COMMENTS:

We have no record of the existing septic
system in our files. The house is vacant and
the septic system is not in use at this time.

Our office hours are Monday through Friday, 8:00 to
4:30. Please contact our office if you have any ques-
tio~ concern~g this report.

ROBERT FOSTER, REGISTERED SANITARIAN

Senator Building 220 High Street NE .
