July 23, 1991 <br /> <br />SUBJECT: <br /> <br />SITE EVAL~T~IO~-S/~ND FILTER APPROVAL <br /> <br /> 85~Holmquist Rd SE T.SS, R1W, Sect31 <br /> Parcel 2 1.50 Acres <br /> <br />As part of the site evaluation of the above described <br />property, it has been determined that the property does <br />not meet the requirements for a standard on-site sewage <br />system. However, the site conditions on the property do <br />qualify for a conventional sand filter alternative system <br />under the requirements of Chapter 340, Section 71-290 of <br />the Department of Environmental Quality On-Site Sewage <br />Disposal Rules. A sand filter system is designed to serve <br />properties where adverse soil conditions exist and a <br />standard septic system could not be expected to function <br />properly. A description of the conditions on the above <br />property is included in the attached site evaluation <br />field worksheet. The following conditions are part of the <br />approval for the installation of a sand filter system on <br />the property. <br /> <br />The property owner is responsible for the continu- <br />ous operation and maintenance of the system. <br />The septic tank must be pumped by a licensed <br />septic tank pumper every four (4) years as part of <br />the system operation and maintenance. The dosing <br />tank should be cleaned at the same time. <br />An inspection of the system is required by this <br />office every four (4) years. An alternative <br />system evaluation fee shall be charged for each <br />inspection. <br /> <br />Plans must be submitted with the permit application and <br />shall include applicable drawings, details and written <br />specifications to fully describe the proposed construc- <br />tion and allow system installation by contractors. <br /> <br />Our office hours are Monday <br />Please contact our office <br />concerning this report. <br /> <br />LEE A.
