The following conditions are part of the
approval for the installation of a sand filter system on
the property.

The property owner is responsible for the continu-
ous operation and maintenance of the system.
The septic tank must be pumped by a licensed
septic tank pumper every four (4) years as part of
the system operation and maintenance. The dosing
tank should be cleaned at the same time.
An inspection of the system is required by this
office every four (4) years. An alternative
system evaluation fee shall be charged for each
inspection.

Plans must be submitted with the permit application and
shall include applicable drawings, details and written
specifications to fully describe the proposed construc-
tion and allow system installation by contractors.

Our office hours are Monday
Please contact our office
concerning this report.

LEE A.
