July 23, 1991 SUBJECT: SITE EVAL~T~IO~-S/~ND FILTER APPROVAL 85~Holmquist Rd SE T.SS, R1W, Sect31 Parcel 2 1.50 Acres As part of the site evaluation of the above described property, it has been determined that the property does not meet the requirements for a standard on-site sewage system. However, the site conditions on the property do qualify for a conventional sand filter alternative system under the requirements of Chapter 340, Section 71-290 of the Department of Environmental Quality On-Site Sewage Disposal Rules. A sand filter system is designed to serve properties where adverse soil conditions exist and a standard septic system could not be expected to function properly. A description of the conditions on the above property is included in the attached site evaluation field worksheet. The following conditions are part of the approval for the installation of a sand filter system on the property. The property owner is responsible for the continu- ous operation and maintenance of the system. The septic tank must be pumped by a licensed septic tank pumper every four (4) years as part of the system operation and maintenance. The dosing tank should be cleaned at the same time. An inspection of the system is required by this office every four (4) years. An alternative system evaluation fee shall be charged for each inspection. Plans must be submitted with the permit application and shall include applicable drawings, details and written specifications to fully describe the proposed construc- tion and allow system installation by contractors.
