The change in design is
from a Concrete basement wall to a basement wall using a polystyrene
product, "CONFORM" forms.

The preliminary review dated October 29, 1992 included a design for a
basement wall with less than fou~ feet in height of unbalanced fill for
a length of less than 12 feet in accordance with Table NO. R-304.3b.

The requirements of this table also apply to this new design using the
CONFORM product. If the height of unbalanced fill is over four feet for
more than 12 feet, an engineered design is r~quired.

The product listing for the CONFORM walls is for foundations and
exterior walls only. Install these forms in accordance with the
manufacturer's installation instructions and ICBO Evaluation Report NO.
4572.

CONFORM walls may be used as retaining and basement walls with
reinforcement provided they ar~ designed in accordance with accepted
engineering principles. No design was submitted for walls with more
than four feet of unbalanced fill.

The te~ted U-value for one of conform forms, with Concrete, is not
equivalent to an R-21 interior insulated wall. The Conform assembly,
with the edge of the slab installed against the insulated concrete wall
form (as detailed on the attached detail of the basement wall), is
energy equivalent to an R-21 interio~ insualted basement wall and will
therefore m~et the R-21 basement requirement per a review by the State
~uilding codes Agency.

Foam plastics shall be separated from the interior of a building by an
approved thaermal barrier of 1/2-inch gypsum wallboard or equivalent
thermal barrier material which will limit the average temperature rise
of the unexposed surface to not more than 250 degrees F. after 15
minute~ of fire exposure. The basement walls shall be protected.

Approval as a result cf this plan review or an inspection shall not be
construed to be an approval of a violation of the provisions of the dwelling
code or of other ordinances of the jurisdiction. Reviews or inspections
p~esuming to give authority to ~iolat~ or cancel the provision~ of the
dwelling code or of other ordinances of the jurisdiction shall not be valid.

sf

"APPROVED" only when this sheet is attached to plans
