<br />LOMR from the FederaJ Emergensy Management Agency (FI=MA) can amend the FIRM and remove the FederaJ requirement for a
<br />lending im~ffution to r~quira the purchase of flood insurance. Note that the tending institution may still m~quire flood insurance.

<br />This certificate is only used to certify the elevation of the reference level of a building. I! a non-residential building is being fleodproofed,

<br />INSTRUCTIONS FOR COMPLETING THE ELEVATION CERTIFICATE

<br />The Elevation CorUScate is to be o=mpleted by a land surveyor, engineer, or architect who is autho~zed by state or IocaJ ~aw to co~y
<br />etev-a~on foforma~len when the elevation information for Zones A1 -A30, A~, AH, A (wffh BFE's), V1 -V30, VE, and V (with BFE's) is
<br />required_ Community offic{~ts who are ~uthorized by kx=al law or ordirk~mce to provide flondplath management information may also
<br />complete this form. For ZoneS AO and A (without BFE's}, a bu~ing of~--~,at, a property owner, or an owner's representative may
<br />provide the information on this certification.

<br />The Elevation Certificate identifies the building, ~s owner and its I~ttiee. Provide the building owner's name(s), the building's complete
<br />~17eet addn~ss, and tot end btec~ number. If the proper~ address is a ~u~fl route or PO box number, provide a ~gal description or an

<br />SECTION B ~ Insurance Rate Map Information
<br />
<br />In order to property complete the Elevation Cer~cate, it is r~cessary to locate the building on the appropriate FIRM, and record the
<br />ap~'cp~ate information. TO obtain a FIRM, contact the c=:;~nmun;,~y or ceJI ~-80G,3.33-1363.

<br />The Elevation Cet~fflcate may be completed based on either the FIRM in effect at the time of the cortificetlon or the FIRM in effect when
<br />const.,'ucfion of the building was st~ed.

<br />Items I - 6. Using the FIRM Index and the apprep~ate FIRM paneJ for the community, recor~ the commun~ number, panel (or page)
<br />number, suffix, end Index date. From the appmpriste FIRM panel, locate the property and record the zone and the BFE (or flood depth
<br />number) at the buiidlng site. BFE's ere shown on a RRM for zones Al-A30, AE, AH, VI-V30, and VE; t~]d depth numbers are shown
<br />for Zone AO.

<br />Item 7. Record the verflca~ datum system to wh~-J~ the elevations on the appiicable FIRM are referenced. The datum is specked in the
<br />upper dght comer of the ~e bles~ of the FIRM.
<br />
<br />Item 8. In A or V Zones where I~FE's are not provided on the FIRM, the community may have established BFE's based on det~ h'om other
<br />sources_ For subd~sions and other development greater than 50 lots er 5 ac~s, establishment of BFE's is mqulred by cemrnun~
<br />flo(x:lplnln management ordinance. When this is the case, complete this item_
