AUMSVILLE, OR 97325 <br /> Sewage Complaint-Failing Septic System <br /> <br />An investigation was made at the above address on April <br />3, 1995 in conjunction with a building permit application <br />at 9338 Silver Falls Hwy SE. Aumsville, Oregon 97325. <br />During the review I noticed that sewage effluent was <br />discharging onto the ground surface from the adjacent <br />residence at 9358 Silver Falls Hwy SE. <br /> <br />The Department of Environmental Quality On-Site Sewage <br />Disposal Rules state that the discharge of sewage or <br />septic tank effluent onto the surface of the ground <br />constitutes a public health hazard and is prohibited. It <br />is, therefore, necessary for repairs to be made to the <br />sewage system at the above address to eliminate the <br />sewage discharge to the ground surface. <br /> <br />An application must be submitted to this office for an <br />On-Site Sewage Disposal Repair Permit along with the <br />required permit fee. Once the permit is issued, the <br />system must be repaired in accordance with the require- <br />ments of the permit. The application must be received by <br />this office within fourteen (14) days from the date of <br />this letter and the repairs must be completed within <br />thirty (30) days from the date of this letter. <br /> <br />If you have any questions, please contact this office. <br />Our office hours are Monday through Friday, 8:00 to 4:30. <br /> <br />LEE A.
