STAYTON, ORE. 97383 <br />Site Number: 95-12694 <br /> <br />An inspection was made at the above address on June 2, <br />1995 to determine if the existing on-site sewage system <br />located on the property appeared adequate to serve the <br />purpose described in the attached application. This <br />application is to connect a new sales office to the <br />existing septic system. While inspecting the property, <br />the following violation was noted which is in conflict <br />with the Department of Environmental Quality On-Site <br />Sewage Disposal Rules. <br /> <br />Sewage is discharging onto the ground surface within <br />the drainfield area. I believe that an effluent pump is <br />used to convey sewage effluent from the septic tank to <br />the drainfield. <br /> <br />Since the above violation exists, we cannot approve the <br />application to use the existing on-site sewage system.
