NE
Aurora, Oregon

PROPERTY OWNER:

Michael Zulaski
20612 Olmstead Rd.
Aurora, Oregon

An inspection was made at the above address on January 28, 1988
~o determine if the existing on-site sewage s~stem located on the
property appeared adequate to serve the purpose described in the
attached application. This application is to place a hardship
mobile home on the property. While inspecting the property, the
following violation was noted which is in conflict with the
Department of Environmental Quality On-Site Sewage Disposal
Rules.

There is gray water discharging to the surface of the ground
on the south side of the house. All plumbing must be
connected to the septic system.

Since the above violation exists, we cannot approve the applica-
tion to use the existing on-site sewage system. It will be
necessary to obtain a plumbing permit and make the necessary
corrections before we can approve the sewage system on this
property. It appears that the=e is insufficient grade to allow
installation of a sewer line from the mobile home to the existing
~eptic tank. If this is the case, it will be necessary to obtain
an on-site sewage permit to install a new septic system.

Our office hours are Monday through
cont/~t our office if you have any

Friday, 8:00 to 4z30.
