SE <br />A%Lmsville, Oregon <br /> <br />18 <br /> <br />PROPERTY OWNER: <br /> <br />Rodney & Maria Coakley <br />185 culver Lane s. <br />Salem, Oregon 97302 <br /> <br />An inspection was made at the above address on April 25~ 1988 to <br />determine if the existing on-site sewage system located on the <br />properly appeared adequate %0 serve the purpose described in the <br />a~taehed application. This application is to replace an existing <br />mobile home. No evidence of failure was found and the system <br />appeared to be functioning in a satisfactory manner. The septic <br />tank was pumped on April 1, 1988 by Stayton Septic Service and <br />the tank was inspected at that time. <br /> <br />This notice is issued with the recommendation that the septic <br />tank be pumped and inspected approximately every five years.
