This
application is to replace an existing hOUse with a
mobile home. NO eVidence of failure was found and the
system appeared to be functioning in a satisfactory
manner. The septic tank was pumped on February 8, 1989
by Stayton Septic Service and the tank was inspected at
that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed
septic tank pumper approximately every five years. This
notice does not guarantee satisfactory oF continuous
operation of the existing on-site sewage disposal
system.

COMMENTS:

we have no record of the existing septic
system in our files. The drainfield appears
to be marginal and may net function under an
increased sewage flow.

Our office hours are Monday through Friday, 8:00 to
4:30. Please contact our office if you have any
ques~his report.
ROBERT F0STER,~R~GISTERED SANITARIAN

~nator Buildln8 220 High street'"~,lE ; Salem, Oregon 97301-3670
