~l Apl~licatlon of Theodore and Theodora Peters to place <br />a mobile home for med£cal hardship purposes oh a ~0~8 &~rs parcel <br />in an b~FU (EXCLUSIVE FARM USE) zone on pr~3~ ~,~.~at 9117 <br />Silver Falls Highway SE, Aumsville (Sec, 13~ TSS~ R2W) Tax Lot <br />7~0-300. <br /> <br />~= Notice is hereby given that the Planning Director for <br />Macinn County has APPROVED t~e above described Conditional Use <br />apgl~cation subject to certain conditions, <br /> <br />This conditional_use permit is valid only when exercised by <br />~~9--~-- The effective perio~.o£ an approved Condtt- <br /> i6nal use ma~ be extended for addltzonal year subject to <br /> <br /> an <br />approval of an extension (Exten~ion form available from the <br />Planning Division). <br /> <br />This permit may be renewed for successive one year periods if the <br />applicant submits to the Planning Department, on an annual basis, <br />a new Physician's Certificate which indi~ates that the hardship <br />continues to exist. <br /> <br />This de~lsion does not inclede approva~ of a building permit. <br /> <br />~ The following conditions must be met before a buildino <br />mermit can be obtatne~ ~r ~b~ aPProved u~e established= <br /> <br />The applicant shall submit a detailed site plan. This plan <br />shall ~nol~de the existing dwelling, septic system, well, and <br />proposed location of the hardship dwelling. The hardship <br />dwell~n9 shall be located as close as posslble to the existing <br />dwelling and shall maintain a m~nimum 100 foot setback from <br />the west property line. This plan shll be reviewed and <br />approved by the Chief ~lanner prior to the issuance of any <br />building permits. <br /> <br />The applicant shall obtain approval fo: subsurface sewage <br />disposal and all other required permits from ~he Ma~ton County <br />Building Inspection Division. <br /> <br />The applicant shall submit a Mobile Home Removal Agreement to <br />the Planning Division. This agreement specifies that <br />placement of the mobile home is temporary and that it will be <br />removed after the hardship ceases to exist (ag=cement <br />enclosed). <br /> <br />The applicant shall sign and submit a Declaratory Statement <br />to the Planning Division. This statement shall be recorded <br />by the applicant with the Marion C~unty Clerk after it has <br />been reviewe~ and signed by the Planning Director (Declarat- <br />ory Statement enclosed). <br /> <br /> <br />
