This application is to replace an existing mobile
home. No evidence of failure was found and the system
appeared to be functioning in a aatisfactory manner.
The septic tank was pumped on October 9, 1989 by
Stay%on septic Service and the tank was insgected at
that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed
septic tank pumper approximately every five years.
