Davidson
P.O. Box 592
Detroit, Oregon

An inspection was made at the above address on February
5, 1990 to determine if the existing on-site sewage
system located on the property appeared adequate to
serve the purpose described in the attached applica-
tion. This application is to place a new mobile home on
an existing site. NO evidence of failure was found and
the system appeared to be functioning in a satisfactory
manner. The septic tank was pumped on September 19,
1989 by Stayton Septic Service and the tank was
inspected at that time.

This notice is issued with the recommendation that the
septic tank be pumped and inspected by a licensed
septic tank pumper approximately every five years. This
notice does not guarantee satisfactory or continuo~s
operation of the existing on-site sewage disposal
system. Planning approval may be required for the
proposed development of the property.

COMMENTS: We have no record of :ha existing septic
system in our files.

Our office ho~rs are Monday through Friday, 8:00 to
4:30. Please contact our office if you have any
questions concerning this report.

ROBERT FOSTER, REGISTF.
