September 15, 1992
APPLICART~ EL~R cHAS~

Your sit~ has been found suitable for a STANDP~D ON-SIT~ ~WAGE DISPOSAL
SYSTEH. The following construction s~eci~ications shall apply to the installa-
tion of your septic system.

1. The septic tank shall have a minimum liquid capacity of 1009
gallons.

2. The projected daily sewage flow for this project is calculated at
~50 qallon~ ~ dav. Based on soil conditions observed on this
site, the disDosal field will be sized at .~50 linea)~f~et Der 150
~allons of projected daily sewage flow. A minimum of ~5.0 lineal
feet of disposal field will be reqlired for your proposed develop-
ment. The disposal ~=enches are constructad two (2) feet wide and
on minimum ten (10} foot centers and with no line ex~ding 12~
feet in length. The trenches and perforated pipe are installed
level (~l") and contoured to follow the natural ground surface.

3. The disposal field shall be ccnst~eted with ~ial distribut~o~
with a minimum trench depth of 24 inc~e~ and a maximum treneh
depth of ~.Q inches. There must be at least 12 inches of backfill
over the top of the gravel, measured from the natural ground
surface.

A groundwater interceptor is =eq~ired as pa~ of this system. This
interceptor trench must be constructed a minimu~n twelve (12) inohe~ wide by 42
inches deep with 36 inches of clean gravel placed over a four (4) inch smooth
wall perforated pipe~ The interceptor must be located at least ten (10) feet
upslope frem the d£s~sal field and must discharge through a watertight~ non-
p~rforated pipe beside or b~lew the disposal field. A separation Of 25 feet is
reqUired from the septic tank if the tank is louated upslo~ of the groundwa-
ter intercepter.

An effluent lift pump may be necessary to construct the disposal field
in the approved area and at the correct depth.

This approval is site specific to :he area tested and the septic system
mupt be located in the immediate area of the test pits unless otherwise
rized by this office. A new site evaluation may be reqUired if the syst~ is
proposed for a diffeFent area on the property.

SPECIAL ~ONDITION$:
The existing lower soil test pit is dug too close to ~he downslope road
cut and ditch. Also, too close to the proposed well. The draiufields
will need to be shifted fa=ther upslope and the well will need ~0 be
relocated or eliminated altc~ethe~ and the residence serviced from an
offsite water ~pply. Refer to the enclose~ setback list which outlines
minimum separation distances between eomponents of the septic systems
a~d othe~ features~ existing and proposed. The new lot lines must me~t a
ten (10} feet mini~az~ setback from the existin~ adjacent drainfi~ld and
the replacement drainfield sites. A prec0nStruCtion field stakeout is
recommended. Refer to the enclosed stakeout procedure to complete the
stakeout.

~ZL A V~LID PERmiT ~ ~ ISSI~,,BY T~I~
