Based on the results of this study, <br />site approval has been granted. This approval is specific to the area tested, and the system <br />must be installed in the immediate area of the test pits. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to.the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 250 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES, MINIMUM; 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL. BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (10) 'foot <br /> centers and with no line exceeding 125 feet in length. The trenches and perforated <br /> pipe are installed level (+/- one inch) and contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 29, <br /> 1976. INSTALL THE REPAIR DRAINFIELD AS FIELD STAKED FOR <br /> LOCATION. AN EFFLUENT PUMP WILL BE NECESSARY TO INSTALL THE <br /> DRAINFIELD LINES IN THE APPROVED AREA AT THE CORRECT DEPTH. <br /> AFTER INSTALLATION, THE DRAINFIELD SITE MUST BE BARRICADED <br /> FOR PROTECTION FROM VEHICLE TRAFFIC. THE OLD TANK MUST BE <br /> PUMPED AND PROPERLY ABANDONED. A PLUMBING PERMIT MAY BE NEC- <br /> ESSARY TO PLUMB ALL WASTE LINES INTO THE NEW SEPTIC TANK. <br /> THERE IS A MODERATE TO SEVERE ROCK HAZARD ASSOCIATED WITH <br /> DRAINFIELD INSTALLATION. REMOVE ROCK LARGER THATN SIX INCHES <br /> FROM BACKFILL MATERIAL. USE STRAW IF NECESSARY TO PROTECT <br /> THE DRAINLINE PIPING. OBSERVE ALL USUAL SETBACK DISTANCES. <br />'2. The attached ORANGE CARD must be posted at the count% road. <br />~. The site must be protected from traffic, livestock, ~arming <br /> or other damaging activities. · <br />4. This system Must be inspected prior to cover. To request <br /> an inspection of completed work, call 588-7?04 (24-hour <br /> line). When requesting the inspection, be sure to giv~ <br /> installer's name, phone nuMber~ size of tank, and total <br /> length of drainfield.
