Based on the results of this study, site <br />approval was granted. The approval is site specific to the area tested, and the septic <br />system must be located in the immediate area of the test pits, unless otherwise authorized <br />by this office. A new site evaluation may be required if the system is proposed for a <br />different area om the property. <br /> <br />'rh~s approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner which would prohibit the issuance o~ a permit. Soils in the approved area <br />shall not be altered by cutting, filling, or other developmemtal activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above items may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records. Show this permit to the septic installer prior to <br />installation of the system. Any deviation from the approved plan must have prior approval from <br />this office. <br /> <br />The following construction specifications shall apply to the installation of the initial <br />septic system: <br /> <br /> TYPE OF INITIAL SYSTEN~ STANDARD <br /> MINIMUM SEPTIC TANK CAPACITY: 1~0~ GALLONS <br /> LINEAL FEET OF ORAINFIELD~ <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH OEPTH: 24INCHES, MINIMUM; 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL ~ACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: Y <br /> <br /> The d~sposal tremches are constructed two (2) feet wide and on minimum ten (lB) foot centers <br /> and with no line exceeding 125 feet in length. The trenches and perforated pipe are <br /> installed level (+/- one inch) and contoured to follow the natural §round surface. <br /> <br /> TYPE OF FUTURE REPLACEMENT SYSTEM: STANDARD <br /> <br />COMMENTS AND CONDITIONS <br /> <br />I. THIS SEPTIC REVIEW NAS COHPLETED BY LEE GRIMES ON JUNE 24, <br /> 1997. INSTALL THE SEPTIC SYSTEM DRAINFIELD LIHES AS FIELD <br /> STAKED FOR LOCATION. REFER TO THE SITE PLAN OF THE STAKEOUT <br /> DATED JUNE 25, 1997. AN EFFLUENT PUMP IS HECESSARY TO IN- <br /> STALL THE DRAINLINES AS STAKED AT THE CORRECT DEPTH. ODSERVE <br /> ALL USUAL SETDACK AND SEPARATION DISTANCES. ALLOW SPACE FOR <br /> THE UP-SLOPE CURTAIN DRAIN. USE CARE NOT TO ALTER THE TOP- <br /> SOIL WITHIN THE DRAINFIELD SITES PRIOR TO INSTALLATION. DRY <br /> SOIL, SUMNER SEASON CONSTRUCTION IS NECESSARY TO PREVENT <br /> SOIL COHPACTIOH DURING INSTALLATION. <br />2. The attached ORANGE CARD must be posted at the county road. <br />3. The site must be protected from traffic, livestock, fareing <br /> or other damaging activities. <br />4.
