Soils in the approved area shall not b~ <br />altered or sodified by cutting~ zilling~ or other developmental activities ~hich ~ould ispede the <br />successful operation of the initial o~ replacement soil absorption systems. Failure to co~plx ~ith <br />the above items may void this approval. <br /> <br />This report is not a persit to install the sewage system. A persit must be obtained before <br />any system construction occurs. A permit may be issued only after the property has been found to <br />be cospatible ~i~h applicable LCDC acknowledged local comprehensive land use plan. A Statesent of <br />La~d Use Compatioility or i~s equivalent is required before a construction per,it can be issued. <br /> <br />A d~tailed site plan. dra~o~to ~cale (e~: 1".= he")~ ~st be. submitted ~ith the permit <br />appiica~ion before the construction-installation permit can ~e issuee. The system must be <br />installed by the property o~ner nv a registered contractor. <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction per,it obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Cosmission Rules. SOIL CONDITIONS AT THE UPPER THREE TEST PITS AREMAR- <br />GINAL TO CONSIDER STANDARD SYSTEM DRAINFIELDS PROTECTED WITH <br />AN UP-SLOPE CURTAIN DRAIN. DRAINFIELDS MUST SE LOCATED WITH- <br />IN THE IMMEDIATE AREA OF THE UPPER PITS AND ND FARTHER DDWN- <br />SLOPE. THE WELL AND HOUSE PROBABLY CANNOT DE LOCATED AS PRO- <br />POSED BECAUSE OF THE DRAINFIELD LOCATIONS. OBSERVE ALL USUAL <br />S-TBACK AND oEPARATION DI TANCES. HOLD OFF SITING THE HOUSE, <br />WELL AND AL[_ EXCAVATION PENDING APPROVAL OF THE INDIVIDUAL <br /> <br /> <br />
