SITE EVALUATION DENIAL DATE/TIME : 07/02/98 08~11 DENIAL NO TYPE : SE STATUS DENIED PAGE 97-08522 DENIED 07/02/1998 SITE EVALUATION DENIAL Ibis property has been evaluated for the feasibility of the installation of a septic system for a RESIDENTIAL building site, Based on the results of this stu~. the site does not qualify for a standard septic system or any of the alternative systems and is DENIED, he conditions on the site do not meet the minimum requirements of the Department of Enviremmental Quality On-Site Sewage Disposal Rules. These rules are specied in Oregon Administrative Rules. Chapter 340. Division 71, Sections 100 - 600, This study was done only in the area proposed for on-site sewage disposal, as indicated on the submitted site plan. Within the boundaries of the parcel, it is possible that soil conditions in a different area of the property may be different than those examined in the test pits. Additional test pits will be inspected, at no additional cost, if a written request and revised site plan is submitted to this office within 90 days of the DENIAL date, If no request is submitted for re-evaluation within the 90 days, the DENIAL will stand. Appeals of this DENIAL must be made in writing directly to the Willa~tte Valley Regional Office of the Department of Environmental Quality (DEQ), and be accompanied by a permit denial review fee, The following conditions outline the specific reasons for the DENIAL~ COMMENTS AND CONDITIONS 1. THIS SEPTIC REVIEW WAS COMPLETED BY THE MARION COUNTY BUILD- ING DEPARTMENT ONSITE WASTE DISPOSAL STAFF OVER THE WINTER AND SPRING OF 1997 AND 1998. THE WATER TABLES OBSERVED WERE T[~) HIGH TO BE APPROVED FOR ANY l~PE OF SEPIC SYSTEM CUR- RENTLY AVAILABLE. 2. THE A~ACHED OP~qNGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected. 4, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and rea~ for inspection, call our Automated Inspection Request line at 273-a~427 to request an inspection, Enter the number "BO1" for a PRE-COVER inspec- tion, You may leave a message for the inspector if desired, Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested An inspection should be completed within seven (7) days.
