THE REVIEW IS TO CONSIDER REPLACING THE COLLAPASED <br /> SEPTIC TANK WITH A NEW 1~0 CONCRETE ~TREET GRADE TANK. THE <br /> NEW TANK WILL BF SET IN THF SAME LOCATION AS THE EXISTING <br /> TANK. THE OLD TANK MUST BE PUMPED AND PROPERLY ABANDONED. <br /> SU~flIT A COPY OF THE PUMPING RECEIPT Afl~ COMPLETED ABANDON- <br /> MENT FORM TO OUR OFFICE UPON CONNECTION TO THE NEW TANK. <br />2. The attached ORANGE CARD must be posted at the county road. <br />3. The s~te must be pro~ect~d from traffic, livestock, farming <br /> or other damaging actlvities. <br />4. This system must be inspected prior to cover. When work is <br /> complete, call our Autometed Inspection Request Line, <br /> at 373-4427, to request an "801" PRE-COVER inspection. <br /> Leave a message for the inspector to indicate the name of <br /> the installer and phone number, the size of tank installed <br /> and the length of drainfield. An inspection should be <br /> completed within seven (7) days. <br /> <br /> <br />
