No evidence of failure was found, and the system appeared to be functioning in a satisfactory manner. The septic tank was pumped 03/12/98 , by FORM GIVEN and was inspected at that time. This notice is issued with the recommendation that the septic tank be pumped and inspected by a licensed septic tank pumper approximately every five years. This notice does not guarantee satisfactory or continuous operation of the existing on-site sewage disposal system. Planning approval may be required for the proposed development of the property. COMMENIS AND CUNDiIIUNS 1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES DN MARCH 19, 1998. THE REVIEW IS TO CONSIDER ADDING ONE BEDROOM TO THE EXISTING TWO BEDROOM HOUSE. IT APP~RS THAT THE NEW FOUND- ATION WILL CONFLICT WITH A PART OF ONE OR MORE SEPTIC SYSTEM DRAINFIELD LINES. THE PARTS OF THE DRAINLINES WITHIN TEN FEET OF THE FOUNDATION MUST BE RELOCATED. SEE SITE PLAN OF 3/20/98. FUTURE SEPTIC SYSTEM REPAIRS MIGHT INCLUDE AN AL- TERNATIVE SAND FILTER SYSTEM, SOIL DRAINAGE AT THE NORTHERLY PORTION OF THE PROPERTY APPEAR SEVERE WITH A WINTER SE~ON WATER TABLE AT OR NEAR THE GROUND SURFACE. SOIL CONDITIONS ON lltE UPPER SURFACE AT THE EXTREME SOUTH PART OF THE PROP- ERTY MIGHT BE BE1-FER IF THERE IS ENOUGH ARF~q FOR DPJNINLINES. 2. THE A1-FACHED DPJNNGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection, The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities, Other activities such as lo~ging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system m~st both be protected. 4, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and reacLv for inspection, call our Automated Inspection Request line ~t 373-4427 to request an inspection.
