No <br />evidence of failure was found, and the system appeared to be functioning in a satisfactory manner. <br />The septic tank was pumped 03/12/98 , by FORM GIVEN and was inspected at that time. <br /> <br /> This notice is issued with the recommendation that the septic tank be pumped and inspected by <br />a licensed septic tank pumper approximately every five years. <br /> <br /> This notice does not guarantee satisfactory or continuous operation of the existing on-site <br />sewage disposal system. Planning approval may be required for the proposed development of the <br />property. <br /> <br />COMMENIS AND CUNDiIIUNS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES DN MARCH 19, <br /> 1998. THE REVIEW IS TO CONSIDER ADDING ONE BEDROOM TO THE <br /> EXISTING TWO BEDROOM HOUSE. IT APP~RS THAT THE NEW FOUND- <br /> ATION WILL CONFLICT WITH A PART OF ONE OR MORE SEPTIC SYSTEM <br /> DRAINFIELD LINES. THE PARTS OF THE DRAINLINES WITHIN TEN <br /> FEET OF THE FOUNDATION MUST BE RELOCATED. SEE SITE PLAN OF <br /> 3/20/98. FUTURE SEPTIC SYSTEM REPAIRS MIGHT INCLUDE AN AL- <br /> TERNATIVE SAND FILTER SYSTEM, SOIL DRAINAGE AT THE NORTHERLY <br /> PORTION OF THE PROPERTY APPEAR SEVERE WITH A WINTER SE~ON <br /> WATER TABLE AT OR NEAR THE GROUND SURFACE. SOIL CONDITIONS <br /> ON lltE UPPER SURFACE AT THE EXTREME SOUTH PART OF THE PROP- <br /> ERTY MIGHT BE BE1-FER IF THERE IS ENOUGH ARF~q FOR DPJNINLINES. <br />2. THE A1-FACHED DPJNNGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as lo~ging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system m~st both be protected. <br />4, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and reacLv for inspection, call our <br /> Automated Inspection Request line ~t 373-4427 to request an <br /> inspection. Enter the number "801 for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired, <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) days. <br /> <br /> <br />
