DATE/TIME
TYPE
OCCUPANCY

AUTHORIZATION APPROVAL
04/27/98 16:14 PERMIT NO
SP STATUS
R-2 ISSUED
TO EXPIRE
PAGE

98-02580
ISSUED
04/27/1998
04/27/1999
2

AUTHORIZATION APPROVAL

This Authorization Notice is for an existing system and an inspection of the system
determined that it appeared adequate to serve the purpose proposed in the application. No
evidence of failure was found, and the system appeared to be functioning in a satisfactory manner.
The septic tank was pumped 11/21/96 , by STAYTON SEPTIC and was inspected at that time.

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years,

This notice does not guarantee satisfactory or continuous operation of the existing on-site
sewage disposal system. Planning approval may be required for the proposed development of the
property.

1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON APRIL 2.
1998. NO CONFLICT OR FAILURE WAS NOTICED DURING THE REVIEW.
OUR RECORDS INDICATE THAT THE SEPTIC SYSTEM WAS INSTALLED
DECEMBER 27. 1976. IT INCLUDES TWO 1000 GALLON TANKS AND TWO
DRAINFIELDS. THE DRAINFIELD AT THE NORTHWEST CORNER OF THE
HOUSE INCLUDES 2-100' LINES AND THE DRAINFIELD AT THE SOUTH-
EAST CORNER INCLUDES 2-100' LINES. WE RECOMMEND THAT THE
TANKS BE PUMPED ON A REGULAR SCHEDULE. ABOUT EVERY FOUR TO
FIVE YE~,RS TO EXTEND THE SYSTEMS USEFUL SERVICE AS LONG AS
POSSIBLE.
2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD
prior to requesting an inspection. The card must be located
near the driveway to the property to assist the inspector in
locating the access onto the site.
3, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities. Other
activities such as logging, excavating or filling, driveway
~lacement or any similar activities may also damage the soil
in the area and adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected.

FTILE
