Based on the results of this study, site evaluation a~proval is oranted. The a~oroval is site specific to the area tested, and the se@tic systeM Mh~t be located ~n the iMMeO[ate area of the test pits, unless otherwise authorized by this office. A new site evaluatio~ May be required if the system is proposed for a different area on the property. This approval is given on the basis that the lot or parcel will not be further partitioned or subdivided and ~hat conditions on this property and on adjacent' properties are not altered in an~ Manner which would prohibit the issuance of a ~erMit. Soils in the approved area shall not be al ered or Modified by cutting~ filling~ or other ~evelopMental activities which would impede the slccessfil operation of the initial or reolaceMent soil absorption sYsteMs. Failure to COMply with the above itbMs May void this approval. - - - - This report is not a permit to install the sewage systeM. A permit Must be obtained before any system construction occurs.
