Based on the results of this study, site evaluation <br />a~proval is oranted. The a~oroval is site specific to the area tested, and the se@tic systeM <br />Mh~t be located ~n the iMMeO[ate area of the test pits, unless otherwise authorized by this <br />office. A new site evaluatio~ May be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and ~hat conditions on this property and on adjacent' properties are not altered in <br />an~ Manner which would prohibit the issuance of a ~erMit. Soils in the approved area shall not be <br />al ered or Modified by cutting~ filling~ or other ~evelopMental activities which would impede the <br />slccessfil operation of the initial or reolaceMent soil absorption sYsteMs. Failure to COMply with <br />the above itbMs May void this approval. - - - - <br /> <br />This report is not a permit to install the sewage systeM. A permit Must be obtained before <br />any system construction occurs. A permit ~ay b~ issued only after' the property has been found to <br />be compatible with applicable i.CDC acknowl_dged local comprehensive land use plan. A Statem~ent of <br />Land Use CoMpatibility or its equivalent is required before a construction permit can be issued. <br /> <br />A detailed site plan. drawn to scale (ex: 1" = ~8"), Must be submitted with the permit <br />application before the construction-installation permit can be issued. The system Must be <br />installed by the property owner or a registered contractor. <br /> <br />Th~s site evaluation report is valid until an on-site sewa e s stem is installed ursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to t~e EnvironMental <br />Quality CoMMission Rules. INSTALL THE DRIANFIELD LINES WITHIN THE IMMEDIATE <br />A~EA OF THE SOIL TEST PITS. THERE IS A MODERATE ROCK HAZARD <br />ASSOCIATED WITH DRAINFIELD CONSTRUOTION. REMOVE LARGE ROCK <br />FROM BACKFILL MATERIAL.. OBSERVE ALL USUAL SETBACK AND SEPAR- <br />ATION DISTANCES. DRY SOIL. SUMMER SEASON CONSTRUCTIONS IS <br />RECOMMENDED TO PREVENT SUIL COMPACTION DURING INSTALLATION. <br />I.~E CARE NOT TO ALTER THN NAT[RAL TOPSOIL WITHIN THE DRAIN- <br />FIELD SITE PRIOR TO COHSTRUCTION.
