A new site evaluation may be required if the system is proposed for a different area on the property, This approval was given on the basis that the lot or parcel will not be further partitioned or subdivided and that conditions on this property and on adjacent properties are not altered in any manner which would prohibit the issuance of a ~ermit. Soils in the approved area shall not be altered by cutting, filling, or other developmental activities which would impede the successful operation of the initial or replacement absorption systems. Failure to comply with the above items may void this approval. The site evaluation approval runs with the land, but the permit is non-transferable and non- refundable. Keep this document with your records, Show this permit to the septic installer prior to installation of the system. Any deviation from the approved plan must have prior approval from this office. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 12. 1998, INSTALL THE DRAINFIELDS WITHIN THE IMMEDIATE AREA OF THE SOIL TEST PITS. NOTE, THE NECESSARY UP-SLOPE CURTIAN DRAIN. REFER TO SITE PLAN DATED RAY 12, 1998. OBSERVE ALL USUAL SETBACK AND SEPARATION DISTANCES. THERE IS A MODERATE ROCK HAZARD ASSOCIATED WITH DRAINFIELD CONSTRUCTION. REMOVE LARGE ROCK FROM BACKFILL MATERIAL. RECOMMEND DRY SOIL CON- STRUCTION TO PREVENT SOIL COMPACTION DURING INSTALLATION. USE CARE NOT TO ALTE THE NATURAL TOPSOIL WITHIN THE DRAIN- FIELD SITES PRIOR TO INSTALLATION. RECOMMEND PRECONSTRUCTION FIELD STAKEOUT TO CONFIRM THE DRAINLINE LOCATIONS FOR BOTH DI~qINFIELD PRIOR TO SITING THE NELL. 2, THE A~ACHED OP~qNGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval.
