A new site evaluation may be required if the system is proposed for a <br />different area on the property, <br /> <br />This approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner which would prohibit the issuance of a ~ermit. Soils in the approved area <br />shall not be altered by cutting, filling, or other developmental activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above items may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records, Show this permit to the septic installer prior to <br />installation of the system. Any deviation from the approved plan must have prior approval from <br />this office. <br /> <br />The following construction specifications shall apply to the installation of the initial <br />septic system: <br /> <br />TYPE OF INITIAL SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: i000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 450 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL ll~ENCH DEPTH: 24INCHES. MINIMUM: 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: Y <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers <br />and withno line exceeding 125 feet in length. The trenches and perforated pipe are <br />installed level (+/- one inch) and contoured to follow the natural ground surface. <br /> <br />I~'PE DF FUTURE REPLACEMENT SYSTEM: STANDARD <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 12. <br /> 1998, INSTALL THE DRAINFIELDS WITHIN THE IMMEDIATE AREA OF <br /> THE SOIL TEST PITS. NOTE, THE NECESSARY UP-SLOPE CURTIAN <br /> DRAIN. REFER TO SITE PLAN DATED RAY 12, 1998. OBSERVE ALL <br /> USUAL SETBACK AND SEPARATION DISTANCES. THERE IS A MODERATE <br /> ROCK HAZARD ASSOCIATED WITH DRAINFIELD CONSTRUCTION. REMOVE <br /> LARGE ROCK FROM BACKFILL MATERIAL. RECOMMEND DRY SOIL CON- <br /> STRUCTION TO PREVENT SOIL COMPACTION DURING INSTALLATION. <br /> USE CARE NOT TO ALTE THE NATURAL TOPSOIL WITHIN THE DRAIN- <br /> FIELD SITES PRIOR TO INSTALLATION. RECOMMEND PRECONSTRUCTION <br /> FIELD STAKEOUT TO CONFIRM THE DRAINLINE LOCATIONS FOR BOTH <br /> DI~qINFIELD PRIOR TO SITING THE NELL. <br />2, THE A~ACHED OP~qNGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> <br /> <br />
