This approval is specific to the area tested, and the system must be installed in the immediate area of the test pits. Soils in the approved area shall not be altered or modified by cutting, filling, or other activities which would impede the successful operation of the septic system. Observe all required setback/separation distances during the installation of the system, Keep this document with your records. Give a copy of this permit to the septic system installer prior to the installation of the system, Any deviation from the approved plan must have prior approval from this office, The following construction specifications shall apply to the installation of the septic system: TYPE OF REPLACEMENT SYSTEM: STANDARD MINIMUM SEPTIC TANK CAPACITY: 1000 C~A[LONS LINEAL FEET OF DRAINFIELD: 450 TYPE OF DISTRIBUTION: SERIAL DISPOSAL TRENCH DEPTH: 24INCHES, MININUM: TOTAL DRAIN MEDIA DEPTH: 12INCHES DRAIN MEDIA UNDER PIPE: 6INCHES MINIMUM SOIL BACKFILL: 12INCHES GROUND WATER INTERCEPTOR: N 24INCHES, MAXIMUM The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers and with no line exceeding 125 feet in length. The trenches and perforated pipe are installed level (+/- one inch) and contoured to follow the natural ground surface, COMMENTS AND CONDITIONS 1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 22, 1998. SOIL CONDITIONS AT THE TEST PITS ARE MARGINAL FOR A STANDARD SYSTEM DRAINFIELD. THE SOIL IS SHALLOW TO BEDROCK, SO THERE IS A MODERATE ROCK HAZARD ASSOCIATED WITH DRAIN- FIELD CONSTRUCTION. REMOVE LARGE ROCK FROM BACKFILL MATERIAL THE EXISTING SEPTIC TANK IS EXCESSIVELY DEEP AND SUBJECT TO GROUNDWATER INFILTRATION, IT MUST BE REPLACED WITH A WATER- TIGHT TANK BECAUSE GROUNDWATER INFILTRATION MIGHT BE THE PRIMARY CAUSE OF THE CURRENT PROBLEM. THE EXISTING DRAIN- FIELD DID NOT APPEAR TO BE AFFECTED BY EXCESSIVE SATURATION. INVESTIGATION TO CONFIRM WHETHER SEWER LINES ARE BLOCKED SHOULD BE COMPLETED PRIOR TO CONSTRUCTING THE MAJOR REPAIR. 2. THE A~ACHED OPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection, The card must be located near the driveway to the property to assist the inspector in locating the access onto the site, 3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities, Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected, 4, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and ready for inspection, call our Automated Inspection Request line at 273-4427 to request an inspection, Enter the number "801" for a PRE-COVER inspec- tion~ You may leave a message for the inspector if desired, Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested An inspection should be completed within seven (7) days.
