Based on the results of this study, <br />site approval has been granted, This approval is specific to the area tested, and the system <br />must be installed in the immediate area of the test pits. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system, <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system, Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: <br /> TYPE OF DISTRIBUTION: EQUAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES, MININUM; 24INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br /> centers and with no line exceeding 125 feet in length. The trenches and perforated <br /> pipe are installed level (+/- one inch) and contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090, You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 232-1987). <br />2. THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 7/15/98. THE <br /> APPLICATION TO MOVE AND REPLACE THE EXISTING TANK IS GRANTED <br /> AND IF ADDITIONAL DRAINFIELD LINES ARE NEEDED APPROVAL IS <br /> GIVEN FORTHAT ADDITIONAL WORK, KEEP THE TRENCHES AS CLOSE <br /> TO THE TRENCH DEPTH DF THE ORIGINAL DRAINFIELD. THE EXIST- <br /> ING SEPTIC TANK MUST BE ABANDONED AND REMOVED OR' FILLED, THE <br /> ATTACHED ABANDONMENT FORM MUST BE SUBMI1-FED AT THE TIME OF <br /> OF THE SYSTEM INSTALLATION, <br />8. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 373-4427 to request an <br /> inspection. Enter the number "801" for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired. <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) days. <br /> <br /> <br />
