An inspection of the system has determined that the system is failing and in need of <br />repair. Based on the results of that inspection, the system is not approved for the use proposed <br />in your application. Site approval has been granted and the property is approved for the <br />installation of a new system. This approval is site specific and must be installed according to <br />the following specifications and the attached site plan. <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system. <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: SEEPAGE TRENCH <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 300 <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 30INCHES. MINIMUM: 30INCHES, MAXIMUM <br /> TOTAL DRAIN MEDIA DEPTH: 18INCHES <br /> DRAIN MEDIA UNDER PIPE: 12INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on m~nimum ten (10) foot <br /> centers. The trenches and pipe are installed level (+/- one qnch) from the box to <br /> the end of the trench and contoured to follow the natural ground surface. <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 11. <br /> 1998. AREA TO SITE REPLACEMENT DP~qINFIELD LINES IS LIMITED <br /> DUE TO SETBACK REQUIREMENTS FROM WEtLS. IF SPACE IS NOT <br /> AVAILABLE TO SITE 450 FEET OF STANDARD DRAINLINE TRENCHES. <br /> 300 FEET OF ALTERNATIVE SEEPAGE TRENCH CAN BE CONSIDERED. <br /> SEEPAGE TRENCHES WILL BE DUG 30 INCHES DEEP WITH 12 INCHES <br /> OF DRAINROCK BELOW THE DISTRIBUTION PIPE P~ATHER THAN THE <br /> NORMAL SIX INCHES OF ROCK. OBSERVE ALL USUAL SETBACK AND <br /> SEPAPJ~TION DISTANCES. AVOID EXISTING DP~AINLINES. PUMP AND <br /> PROPERLY ABANDON THE OLD SEPTIC TANK. SUBMIT A COPY OF THE <br /> PUMPING RECEIPT AND COMPLETED ABANDNMENT FORM TO OUR OFFICE. <br />2. THE A~ACHED OR,GE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />2. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />4. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 372-4427 to request an <br /> inspection. Enter the number "801" for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired. <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An ~nspection should be completed within seven (7) days. <br /> <br /> <br />
