Based on the results of this study, the site does n~t <br />~ualify for a standard septic system or any of the alternative systems and is DENIED, <br />he conditions on the site do not meet the minimum requirements of the Department of <br />Environmental Quality On-Site Sewage Disposal Rules, These rules are specied in Oregon <br />Administrative Rules, Chapter 340, Division 71. Sections 100 - 600. <br /> <br />This study was done only in the area proposed for on-site sewage disposal, as indicated on <br />the submitted site plan. Within the boundaries of the parcel, it is possible that soil <br />conditions in a different area of the property may be different than those examined in <br />the test pits, <br /> <br />Additional test pits will be inspected, at no additional cost, if a written request and <br />revised site plan is submitted to this office within 90 days of the DENIAL date, If <br />no request is submitted for re-evaluation within the 90 days, the DENIAL will stand. <br /> <br />(A~fpeals of this DENIAL must be made in writing directly to the Willamette Valley Regional <br />lice of the Department of Environmental Quality (DEQ). and be accompanied by a <br />permit denial review fee, <br /> <br />The following conditions outline the specific reasons for the DENIAL: <br /> <br /> COMMENTS AND CONDITIONS <br /> 1. IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center, Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090, You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 232-1987). <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON JULY 22. <br /> I998. THE NECESSARY TEST PITS ARE NOT DUG SO I WAS UNABLE TO <br /> EXAMINE THE SOIL PROFILE WITHIN THE PROPOSED SEPTIC SYSTEM <br /> DRAINFIELD SITES. HOWEVER, IT DOES NOT APPEAR MAT THERE IS <br /> ENOUGH AREA WITHIN THE BOUNDARIES OF THIS PARCEL TO SITE THE <br /> APPROPRIATE DRAINFIELDS, THE APPLICATION FOR ON-SITE SEWAGE <br /> DISPOSAL SYSTEMS MUST BE DENIED. YOU MAY APPLY FOR RE-EVAL- <br /> UATION WITHIN NINTY DAYS OF THIS LETTER AT NOT ADDITIONAL <br /> FEE. BUT THE TEST PITS ARE NECESSARY AND A DETAILED FIELD <br /> STAKEOUT OF ALL PROPOSED DEVELOPMENT IS REQUIRED, REFER TO <br /> THE STAKEOUT PROCEDURE AND THE SETBACK LIST TO COMPLETE THE <br /> STAKEOUT, SUBMIT A TO-SCALE-DRAWN DIAGRAM OF THE STAKEOUT, <br />3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br /> <br /> <br />
