SITE EVALUATION DENIAL DATE/TIME : 07/28/98 11:21 DENIAL NO TYPE : SESP STATUS DENIED PAGE 98-05858 DENIED 07/28/1998 2 SITE EVALUATION DENIAL This property has been evaluated for the feasibility of the installation of a septic system for a RESIDENTIAL building site. Based on the results of this study, the site does n~t ~ualify for a standard septic system or any of the alternative systems and is DENIED, he conditions on the site do not meet the minimum requirements of the Department of Environmental Quality On-Site Sewage Disposal Rules, These rules are specied in Oregon Administrative Rules, Chapter 340, Division 71. Sections 100 - 600. This study was done only in the area proposed for on-site sewage disposal, as indicated on the submitted site plan. Within the boundaries of the parcel, it is possible that soil conditions in a different area of the property may be different than those examined in the test pits, Additional test pits will be inspected, at no additional cost, if a written request and revised site plan is submitted to this office within 90 days of the DENIAL date, If no request is submitted for re-evaluation within the 90 days, the DENIAL will stand. (A~fpeals of this DENIAL must be made in writing directly to the Willamette Valley Regional lice of the Department of Environmental Quality (DEQ). and be accompanied by a permit denial review fee, The following conditions outline the specific reasons for the DENIAL: COMMENTS AND CONDITIONS 1. THE NECESSARY TEST PITS ARE NOT DUG SO I WAS UNABLE TO EXAMINE THE SOIL PROFILE WITHIN THE PROPOSED SEPTIC SYSTEM DRAINFIELD SITES. HOWEVER, IT DOES NOT APPEAR MAT THERE IS ENOUGH AREA WITHIN THE BOUNDARIES OF THIS PARCEL TO SITE THE APPROPRIATE DRAINFIELDS, THE APPLICATION FOR ON-SITE SEWAGE DISPOSAL SYSTEMS MUST BE DENIED. YOU MAY APPLY FOR RE-EVAL- UATION WITHIN NINTY DAYS OF THIS LETTER AT NOT ADDITIONAL FEE. BUT THE TEST PITS ARE NECESSARY AND A DETAILED FIELD STAKEOUT OF ALL PROPOSED DEVELOPMENT IS REQUIRED, REFER TO THE STAKEOUT PROCEDURE AND THE SETBACK LIST TO COMPLETE THE STAKEOUT, SUBMIT A TO-SCALE-DRAWN DIAGRAM OF THE STAKEOUT, 3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected.
