Based on the results of this stud~v, <br />site approval has been granted. This approval is specific to the area tested, and the system <br />must be installed in the immediate area of the test pits, <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system, <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br /> MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DPJNINFIELD: 375 <br /> TYPE OF DISTRIBHTION: EQUAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES. MININUM; 24INCHES, MAXIMUM <br /> TOTAL D~IN MEDIA DEPTH: 12INCHES <br /> DPJNIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (lO) foot <br /> centers and with no line exceeding 125 feet in length. The trenches and perforated <br /> pipe are installed level (+/- one inch) and contoured to follow the natural ground <br /> surface. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1, CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> AIYENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. THIS SEPTIC REVIEW WAS COMPLETED BY Z,EWING ON 8/17/98, THE <br /> SITE IS APPROVED FOR A STANDARD DRAINFIELD REPLACEMENT, THE <br /> UPPER AREA IF USED WILL REQUIRE A PUMP AND EQUAL DIST. THE <br /> LOWER SITE MAY BE USED WITH SERIAL DISTRIBUTION. A 50' SET- <br /> BACK IS REQUIRED FROM THE SEASONAL STREAM BELOW. CAREFULLY <br /> CHECK THE GRADE FROM THE EXISTING TANK TO SEE IF THE PUMP <br /> CAN BE AVOIDED AND KEEPING THE DPJNINFIELD HIGH ENOUGH AND <br /> SHALLOW ENOUGH AND STILL MAINTAIN THE 50' SETBACK. CALL IF <br /> THERE ARE ANY QUESTIONS ON THE INSTALLATION. IF THE EXIST- <br /> ING SEPTIC TANK IS USED WITH A PUMP SYSTEM. IT SHOULD BE <br /> CAREFULLY CHECKED TO ASSURE IT IS WATER TIGHT TO AVOID IN- <br /> FILTRATION. <br />3. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 373-4427 to request an <br /> inspection.
