Based on the results of this study, site <br />approval was granted. The approval is site specific to the area tested, and the septic <br />system must be located in the immediate are~ of the t~st p!~s, unless otherwise,a~thorized <br />by this office. A new site evaluation may De requireo if ~ne system is proposeo TOP a <br />different area on the property. <br /> <br />This approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner which would prohibit the issuance of a permit. Soils in the approved area <br />shall not be altered by cutting, filling, or other developmental activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above iten~ may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records. Show this permit to the septic installer prior to <br />installation of the system. THE <br /> OLD TANK MUST BE PUMPED AND PROPERLY ABANDONED UPON CONNECT- <br /> ION TO THE NEW TANK. SUBMIT A COPY OF THE PUMPING RECEIPT <br /> AND CONPLETED ABANDONMENT FORM TO OUR OFFICE. THE A1-FACHED OPJkNGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities.
