SITE EVALUATION APPROVAL <br />DATE/TIME : 09/29/98 14~39 APPROVAL NO <br />TYPE : SE STATUS <br /> APPROVED <br /> PAGE <br /> <br />98-07746 <br />APPROVED <br />09/29/1998 <br />2 <br /> <br />SITE EVALUATION APPROVAL <br /> <br />This property has been evaluated for the feasibility of the installation of a septic <br />system for a COMMERCIAL building site. Based on the results of this stucty, site evaluation <br />approval is granted. The approval is site specific to the area tested, and the septic system <br />must be located in the immediate area of the test pits. unless otherwise authorized by this <br />office. A new site evaluation may be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems. Failure to comply with <br />the above items may void this approval. <br /> <br />This report is not a permit to install the sewage system. A permit must be obtained before <br />any system construction occurs, A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued. <br /> <br />A detailed site plan. drawn to scale (ex: 1 : 60 ), must be submitted with the permit <br />application before the construction-installation permit can be issued, The system must be <br />installed by the property owner or a registered contractor. <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules. Subject to the above, this report runs with the land and will automat- <br />ically benefit subsequent owners. <br /> <br />Site conditions will require the following installation specifications: <br /> <br />TYPE OF INITIAL SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br />LINEAL FEET OF DRAINFIELD: 150 <br />TYPE OF DISTRIBUTION: EQUAL <br />DISPOSAL TRENCH DEPTH: 18INCHES. MINIMUM: 18INCHES, MAXIMUM <br />TOTAL DRAIN MEDIA DEPTH: 12 <br />DPJNIN MEDIA UNDER PIPE: 6 <br />MINIMUM SOIL BACKFILL: 12 <br />GROUND WATER INTERCEPTOR: N <br /> <br /> TYPE OF FUTURE REPLACEMENT SYSTEM: STANDARD <br /> <br />Before any installation takes place, a permit must first be obtained from Marion County. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090, You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 222-1987). <br />2. THIS SEPTIC REVIEW WAS COMPLIED BY Z,EWING ON 9/22/98, THE <br /> SITE IS MARGINAL FOR THE INST~LATION OF A STANDARD SEPTIC <br /> SYSTEM IN THE AREA PROPOSED, THE DP~AINFIELD IS SIZED FOR 150 <br /> GALLONS PER DAY WHICH WILL ACCOMODATE UP TO 10 EMPLOYEES. <br />2. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUN~ ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4.
