DATE/TIME
TYPE
OCCUPANCY

AUTHORIZATION APPROVAL
09/28/98 15:43 PERMIT NO
SP STATUS
R-2 ISSUED
TO EXPIRE
PAGE

98-08043
ISSUED
09/28/1998
09/28/1999
2

AUTHORIZATION APPROVAL

This Authorization Notice is for an existing system and an inspection of the system
determined that it appeared adequate to serve the purpose proposed in the application, No
evidence of failure was found, and the system appeared to be functioning in a satisfactory manner.
The septic tank was pumped / / . by and was inspected at that time.

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years.

This notice does not guarantee satisfactory or continuous operation of the existing on-site
sewage disposal system, Planning approval may be required for the proposed development of the
property.

CUMMENIS AND CONDIIIUNS

1. CALL BEFORE YOU DIG .... IT'S THE LAW,
A1-FENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center. Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090. You
may obtain copies of the rules by calling the center,
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (502) 232-1987),
2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON SEPTEMBER
25. 1998. THIS REVIEW IS TO CONSIDER CONNECTING A TEMPORARY
HARDSHIP HOME INTO THE SEPTIC SYSTEM WHICH SERVICES THE HOME
AT 8561 LI1-FLE ROAD SE, AOMSVILLE, OUR RECORDS INDICATE THAT
THE PRESSURIZED SEPTIC SYSTEM WAS INSTALLED NOVEMBER 4. 1998
AS A M/L]OR REPAIR. THE SYSTEM WAS OPERATING WITHOUT EVIDENCE
OF ANY FAILURE DURING THE REVIEW. WE HAVE NO OBJECTION TO
THE PROPOSED TEMPORARY HARDSHIP CONNEDTION. THE SEPTIC TANK
SHOULD BE PUMPED ON A REGULAR SCHEDULE, ABOUT EVERY FOUR
YEARS TO EXTEND THE SYSTEM'S USEFUL SERVICE AS LONG AS
POSSIBLE,
D. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD
prior to requesting an inspection. The card must be located
near the driveway to the property to assist the inspector in
locating the access onto the site,
4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities. Other
activities such as logging, excavating or filling, driveway
placement or any similar activities may also damage the soil
in the area and adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected,
5. The septic tank should be pumped and inspected by a licensed
septic tank pumper approximately every five (5) years, The
septic tank is a reservoir for the solid materials in the
sewage waste stream. Excessive accumulation of these solid
materials in the septic tank will cause the solids to wash
out of the tank and into the drainfiled. This will cause
irreversible damage to the drainfield and will eventually
lead to a system failure,
