DATE/TIME
TYPE
OCCUPANCY

AUTHORIZATION APPROVAL
11/12/98 15:02 PERMIT NO
SP STATUS
R-3 ISSUED
TO EXPIRE
PAGE

98-09224
ISSUED
11/12/1998
11/12/1999
2

AUTHORIZATION APPROVAL

This Authorization Notice is for an existing system and an inspection of the system
determined that it appeared adequate to serve the purpose proposed in the application. No
evidence of failure was found, and the system appeared to be functioning in a satisfactory manner.
The septic tank was pumped 3 /20/98 , by VALLEY SEPTIC and was inspected at that time.

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years,

This notice does not guarantee satisfactory or continuous operation of the existing on-site
sewage disposal system. Planning approval may be required for the proposed development of the
property.

1, CALL BEFORE YOU DIG .... IT'S THE LAW.
A~ENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center, Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090, You
may obtain copies of the rules by calling the center.
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (502) 222-1987),
2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON NOVEMBER
13. 1998, REFER TO THE REVISED SITE PLAN DATED NOVEMBER 13.
1998. THE DRIVEWAY TO THE NEW HOME HAS BEEN MOVED TO CLEAR
THE EXISTING SEPTIC SYSTEM DP~AINFIELD LINES, OUR RECORDS
INDICATE THAT THE EXISTING SYSTEM WAS INSTALLED AUGUST 22.
1975. IT INCLUDES A 1450 GALLON CONCRETE SEPTIC TANK AND
TWO 80 FEET LONG DRAINLINES. THE SYSTEM WAS OPERATING
NORMALLY WITHOUT EVIDENCE OF ANY FAILURE DURING THE ONSITE
REVIEW CONDUCTED NOVEMBER 5. 1998. SPACE IS LIMITED FOR
FUTURE REPAIRS WHICH MIGHT INCLUDE AN ALTERNATIVE SAND
FILTER SYSTEM. THE TANK SHOULD BE PUMPED ABOUT EVERY FOUR
TO FIVE YEARS TO EXTEND THE SYSTEMS USE AS LONG AS POSSIBLE.
3. THE Al-~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD
prior to requesting an inspection, The card must be located
near the driveway to the property to assist the inspector in
locating the access onto the site.
4. THE APPROVED SEPTIC SYSTEM ARF~q MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities. Other
activities such as logging, excavating or filling, driveway
placement or any similar activities may also damage the soil
in the area and adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected.

FILE
