Based on the results of this study. site approval has been granted. This approval is specific to the area tested, and the system must be installed in the i~mediate area of the test pits. Soils in the approved area shall not be altered or modified by cutting, filling, or other activities which would impede the successful operation of the septic system. Observe all required setback/separation distances during the installation of the system. Keep this document with your records. Give a copy of this permit to the septic system installer prior to the installation of the system. Any deviation from the approved plan must have prior approval from this office. The following construction specifications shall apply to the installation of the septic system: TYPE OF REPLACEMENT SYSTEM: STANDARD MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS LINEAL FEET OF DP~qINFIELD: 275 TYPE OF DISTRIBUTION: SERIAL DISPOSAL TRENCH DEPTH: 24INCHES. MAXIMUM TOTAL DRAIN MEDIA DEPTH: 12INCHES DRAIN MEDIA UNDER PIPE: 6INCHES MINIMUM SOIL BACKFILL: 12INCHES GROUND WATER INTERCEPTOR: N The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers and with no line exceeding 125 feet in length, The trenches and perforated pipe are installed level (+/- one inch) and contoured to follow the natural ground surface. THIS SEPTIC REVIEW WAS COMPLETED BY Z,EWING ON 10/28/98, THE SITE IS APPROVED FOR A STANDARD SEPTIC SYSTEM REPLACEMENT, AN EFFLUENT PUMP IS NECESSARY TO GET TO THE PROPOSED DP~IN- FIELD AREA AT THE REQUIRED TRENCH DEPTHS, THE EXISTING TANK NEEDS TO BE PUMPED AND ABANDONED, SUBMIT THE A1-FACHED AB- ANDONME~F~ FORM AT THE TIME OF SYSTEM INSTALLATION. 2. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site, 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may al so damage the soil in the area and adversely affect the septic system approval. The areas of the initial septic system and the future repair system must both be protected. 5, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and ready for inspection, call our Automated Inspection Request l~ne at 272-4427 to request an inspection, Enter the number '801" for a PRE-COVER inspec- tion. You may leave a message for the inspector if desired. Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested An inspection should be completed within seven (7) days, 6.
