SITE EVALUATION APPROVAL <br />DATE/TIME : 04/06/00 13:40 APPROVAL NO <br />TYPE : SE STATUS <br /> APPROVED <br /> PAGE <br /> <br />99-02196 <br />APPROVED <br />04/06/2000 <br />2 <br /> <br />SITE EVALUATION APPROVAL <br /> <br />This property has been evaluated for the feasibility of the installation of a septic <br />system for a COMMERCIAL building site, Based on the results of this study, site evaluation <br />approval is granted. The approval is site specific to the area tested, and the septic system <br />must be located in the immediate area of the test pits, unless otherwise authorized by this <br />office. A new site evaluation may be required if the'system is proposed for a different <br />area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems, Failure to comply with <br />the above items may void this approval, <br /> <br />This report is not a permit to install the sewage system. A permit must be obtained before <br />any system construction occurs, A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued, <br /> <br />A detailed site plan, drawn to scale (ex: 1" : 60"), must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property owner or a registered contractor, <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules. Subject to the above, this report runs with the land and will automat- <br />ically benefit subsequent owners. <br /> <br />Site conditions will require the following installation specifications: <br /> <br />TYPE OF INITIAL SYSTEM: SAND FILTER <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br />LINEAL FEET OF DRAINFIELD: 160 <br />TYPE OF DISTRIBUTION: SERIAL <br />DISPOSAL TRENCH DEPTH: 24INCHES, MINIMUM: 24INCHES, MAXIMUM <br />TOTAL DP~qIN MEDIA DEPTH: 12 <br />DRAIN MEDIA UNDER PIPE: 6 <br />MINIMUM SOIL BACKFILL: 12 <br />GROUND WATER INTERCEPTOR: N <br /> <br /> TYPE OF FUTURE REPLACEMENT SYSTEM: SAND FILTER <br /> <br />Before any installation takes place, a permit must first be obtained from Marion County. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1, CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 232-1987). <br />2. THIS SEPTIC REVIEW NAS COMPLETED BY Z.EWING ON 2/31/00, THE <br /> PROPERTY IS APPROVED FOR A SAND FILTER SEPTIC SYSTEM IN THE <br /> AREA OF THE HIGHER TEST HOLES PROVIDED FOR EVALUATION. AVOID <br /> THE NORTHERLY TEST HOLE WITH THE DRAINFIELD, A FIELD STAKE- <br /> OUT AND SAND FILTER PLANS WILL BE REQUIRED PRIOR THE THE tS: <br /> SUANCE OF THE SEPTIC SYSTEM PERMIT, <br />3. THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br /> <br /> <br />
